Withdrawing a Student from Westlake
Families who plan to withdraw a student must complete the Opt-Out form below in order to officially withdraw. Please note that a phone call or email is not sufficient notice. Financial payments will not be adjusted unless the Opt-Out form is signed and submitted.
This form should be used in the following circumstances:
- A student is withdrawing mid-year (indicate the exact last day of attendance).
- A student will not be returning for the following school year (The deadline to withdraw without making a non-refundable down payment for the new school year is March 31).
This form is NOT required in the following circumstances:
- Your child is graduating from high school this year.
- You have completed the Pre-K Commitment Form or Kindergarten Decision Form and indicated that your child will not return.
